Action
Administer:
- Oral colchicine: 1.2 mg, then 0.6 mg 1 hr later (Class II)
- Corticosteroids (Gram stain negative):
- Systemic (Class II):
- Prednisone 40 mg PO daily × 3 days (taper over 1-2 wk) or
- Triamcinolone 60 mg IM × 1, or
- Methylprednisolone 125 mg IV × 1
- Intra-articular (Class II):
- Knee: triamcinolone 20-40 mg × 1
- Smaller joint: triamcinolone 5-10 mg × 1
Also
- Discontinue diuretics when able (Class II)
- Provide alcohol and dietary counseling
- Follow up with rheumatology
