Is ongoing bleeding present?
Background
Laboratory studies are generally not helpful and do not routinely change management, except that an international normalized ratio should be obtained in patients taking warfarin with uncontrolled oral hemorrhage if reversal is planned. Patients with significant, continued bleeding, as well as anticoagulated patients with persistent bleeding, require ED transfer.
Evidence-based medicine requires a critical appraisal of the literature based upon study methodology and number of subjects. Not all references are equally robust. The findings of a large, prospective, randomized, and blinded trial should carry more weight than a case report.
